Responsibilities: As a crucial part of an exceptional treatment team that includes physicians, psychiatrists, therapists, CNA’s and addiction counselors, RNs establish and document nursing diagnoses to serve as a basis for each patient’s plan of care, and aid in creating an environment conducive to each patient’s health and safety, through collecting and charting data by observing, examining, and interviewing patients.

• Establish and document nursing diagnosis, which serve as a basis for the plan of care.

• Develop and modify the plan of care based on assessment and nursing diagnosis.

• Implement the plan of care

• Document nursing interventions and responses to care.

• Communicate nursing interventions and responses to care of other members of the health team.

• Provide client advocacy.

• Be a monitor for nurses

Qualifications:

• Current Oregon RN License

• CPR – Basic Life Support Certification

Education and Experience:

· High School Diploma or higher.

· 1 Year in Mental Health or Detox Preferred

• Associate Degree in Nursing
